Well, Precision doesn't affect your attack bonus, it gives a +5% chance to hit. Attack bonus is used in a formula to determine your % chance to hit, and then Precision adds on to that.
From http://ddowiki.com/page/Armor_Class:
So Precision's 5% bonus would get added in there along with the 20% proficiency bonus. The final computed % chance to hit won't exceed 95%, to maintain a 5% chance to miss.Player's chance to hit: (Player's Attack Bonus + 10.5) / (Target's Armor Class * 2) + 20% proficiency bonus, rounded to nearest 5%
In short the only way to verify at Precision is working is to conduct combat tests and tally your hit rate over a large number of trials with and without Precision.

Offensive Combat Stance: While using Precision mode, you gain +5% to hit and reduce the target's fortification against your attacks by 25%.
Cannot be used while raged.
My rogue's precision stance stays on when he uses a rage potion, when the spell or primal scream is cast on him, and when relentless fury from the helm procs.
Switching it on and off doesn't seem to affect the visible to hit bonus on the inventory tab or combat rolls. I think that is by design, not because it isn't working.

Correct.
Precision doesn't technically add to your attack bonus; it's a flat bonus on the other side of the equation:
To-Hit Chance = (AttackBonus + 10.5) / (EnemyAC * 2) + 20% proficiency + 5% precision
Because of this, it's not mathematically possible to reflect the precision bonus in the to-hit numbers on your weapon display in inventory. You can solve the equation for how much AttackBonus the +5% precision bonus works out to, but that value changes (drastically) depending on what AC you're attacking.
