What is the greatest accomplishment you've achieved in DDO?

I have managed to play this long with never having level capped a char.

Grotesquely violating lepidoptera aside, I'd have to say my greatest accomplishment has been soloing elite Proof is in the Poison with a third level monk. I know - It's not an exceptional accomplishment, but it made me happy.

I lead an EH FoT with no mic and tanked the Stormreaver wearing only medium dragon scale on an arty speced as a generalist and survived. I used a mostly upgraded Nightmare with the Chimera's Breath rune arm, Fabricator's Gauntlets, Ring of Sheltering, Ring of Dejinn, and Tensor's Transformation.

I once went through the entire Blockade Buster quest on Elite without letting the Droaam soldiers raise an alarm.
Just a random, forgettable NPC doing things an NPC does
Things that NPCs do include, but are not limited to:

Having a gold goblet over my head to indicate availability of a quest
Having a catchphrase that never really catches on
Having various rewards that are generally not worth the trouble
    Wandering around randomly
Giving out obvious information if it wasn't obvious enough
Repeating the same lines over and over again

    The greatest accomplishment?

Making the lasting friendships with people I met in game. Everything else is gravy

Finally completing The Pit solo (with the aid of a walk-through guide). And it helped that my character was level 24 by then too.

Garamol solo with a level 17 monk. Got my Icy also, nice reward. And had no trouble leaving a trail of death through the Sub. Felt great, as I always feared going down there.

My personal achievement for which I am most proud: Founding leader of the guild The Fellowship of Argonnessen that is still going without me as I hoped (early on I saw many guilds die if the founding leader left and I tried my best to build my first guild to outlast the leaders). Also proud of the current guild Order of Sunlit Rose that I founded and run and the people who play in it.

    Default Nothing earthshaking

But to this day, my greatest accomplishment was getting Black Dragonscale armor of my forever TR character. Nothing really special to most but I am happy with it.

Greatest accomplishment in DDO: Pulling a Litany in my first ever Abbot run...and putting it up for roll. God **** that was hard.

See, I have this policy that if I pull something named on my first run of a raid (as a kind of 'thank you' for people teaching me), I put it up for roll. On my first FOT I put my comms up for roll, on CitW I put Antipodes up for roll, but that Litany, that was hard to do

To have been able to join an awesome and fun group of players to play and raid with, comprised of various nationalities from all over the world. They have made this D&D experience extremely fun and worthwhile.
