One of Ataraxia rares is scorpion named Schenker. Rudolf Schenker is a guitarist for the heavy metal band SCORPIONS.
End boss in "Fleshmaker's Laboratory" is named Mentau (pronounced MEN-toh) the Fleshmaker. Which is a double pun -- on "Mentos the Freshmaker" and "Doctor Demento" (he makes monsters out of body parts).
"Made to Order" end boss is a marut -- i.e. killer construct who implacably pursues his intented target. His name is ARN-01D. Need I say more?
"Let Sleeping Dust Lie" end boss is named Kkomglol -- combination of three very common texting shorts: "kk", "omg" and "lol".
Droaam army cook (gigantic fat orc) and interrogator are named, respectively, Skinny and Major Pain. I suppose those are obvious jokes rather than easter eggs.
"Fear Factory" has an optional locked room where you fight a party of undead adventurers. They are the characters from 1980's cartoon "Dungeons & Dragons". (Now we know what happened to them!)
Fire elemental end boss of "Rainbow in the Dark" is Turun-Palo, which is Finnish and means The Great Fire of Turku.

Six bosses in "Sins of Attrition" are modeled on six out of Seven Deadly Sins: Gluttony, Sloth, Wrath, Envy, Pride and Avarice, and each has an appropriately themed room (for some strange reason, Turbine omitted Lust...). The real easter egg IMO is the red-named orthon boss you fight fairly early in the quest, Saligia -- SALIGIA is a mnemonic based on the first letters in Latin of the seven deadly sins: superbia, avaritia, luxuria, invidia, gula, ira, acedia.

Surprised the obvious ones from the Vale haven't been mentioned - the multiple quest names that are either song/band names, or really close to them.
Depths series House D, The quest givers are the Manchester United midfield from about the late 1990's.

Depths Series, one guy starts with something about 10 rat tails, this is a reference to another game, where you got them as a reward or something.

My personal favorite.
Monks in PnP D&D didn't use ki or the elemental strikes. These were added to give the class a bit more functionality. Air, Earth, Fire, Water & Void are the five elements from oriental culture, and the Oriental Adventures series for DDO. Ki is from the Ninja class from Complete Adventurer. They combined some of these concepts and created the elemental system for Monks, and it works nicely. Beyond that, the Monk class is almost exactly was it was in PnP (with a few minor tweaks for the system shift).
All of this means that the Finishers are a new creation for DDO, but there's one in particular that I want to highlight.
While the rest of the Monk class abilities gained upon level up are exactly like they were in PnP, their level 20 ability called Shining Star never existed.
Shining Star: Earth:Air:Fire Finisher - You use the combined power of Earth, Wind, and Fire to set up harmonic vibrations within your enemy that force them to dance. A successful Will save negates this effect. (DC 10 + Monk Level + Charisma mod)
Shining Star, performed by Earth Wind & Fire and if you fail your save, you can't stop dancing.
Friggin' hilarious.
Side note: Every time I see an Ogre dancing, this song is playing in my head. Every single time.
