Fens named gear only appearing on elite+ runs?

As title—not seeing them appear on lesser runs in end reward. Why this change?

Oh and Claw of Vulkoor scorpions do not crawl up your leg visually but you get the message that they do and can procees

Probably not an actual removal, but drop rates are reduced on hard/nornal/casual.
