I'd love to see other inevitables! We have the Maruts, but there are also Kolyaruts and Zelekhuts. They are Eberron specific, so DDO is their only hope of getting some screen time.
Well, it's a toss-up between "one big baddie" that's optional (sim to Jubilex, but actually a danger to a party), and "oh no, another of those guys..."...
The Tarrasque, obviously, would be a holy grail of sorts, "the high bar" that would need an ultimately strong and well coordinated and perhaps even specialized party to drop. No mere "big DPS", but strategy, tactics, teamwork, and maybe some luck.
Alternately, a demilich - a true demilich, not some watered-down version - would be a similar challenge.
Heh, or Orcus - always fun times.
Putting a chest w/ some nice named item would be the draw, these should not be part of the quest - so no one can complain about them being "too strong" to complete the quest.
But for lots of smaller headaches, something tricky and dangerous - maybe...
A group of true Illusionists, with Mirror Image, a true Displacement (aim your attack "here", but they're "over there"), monsters that may or may not be "real", etc. Illusionists, when done well, were always one of the biggest headaches in the game (and a static NPC encounter can do nifty things that simply would not work w/ Player Characters).
Metallic dragons, because who doesn't want more dragons?

I was going to support the pro flumph movement but the point about the Terrasque...
Classic pinnacle monster.
The quest ends, not with you killing the Terrasque, but simply being able to survive it by running through the village or city it's trampling- avoiding dead ends (Falling towers) killing invading monsters that try to stop you asap, and surviving for ten minutes. You get out, unflattened, receive the reward and recall, another city flattened and the Terrasque lumbering off into the sunset.
Not every quest is intended to be defeated- some you merely try and survive...
