Bloody Footprints

So, I'm missing something super obvious here, but need a little help.

5009 attained, talked to Mortality and he gave me a slap on the back. However in zipping thru his 4 real options, none award me anything obvious.

I received the cloak for 20. Put it in its slot, great.

The eyes and way more importantly, the Bloody Footprints don't auto acquire when I discuss the first option about favor twice.

Like I said, I'm sure there is a no brainer toggle or what not I'm missing. Please advise.
