I actually leapt off a cliff to escape a pack of dire wolves and hobgoblins in the Tangleroot quest where you have to find the battle orders, etc. The last one is on that overlook (past the traps). I got surrounded by several hobgloblins and apparently some dire wolves had spotted me as I made my way there, because while I was trying cleave my way out about 3 of them joined in with the hobbies and I got knocked down. I am watching my health go down while I lie helpless as fangs tear my limbs and swords buffet my face, I manage to stand back up twice but couldn't even get a swing in before getting knocked down again. I starting spamming the "jump" key while aiming for the cliff, despite dangerously low health and no feather fall. Finally managed to jump out of the ring of death, land at the bottom with like 2 health left, and proceed to quaff csw pots like it was sweet tea in the south. Good times!

Ordinary humans have inhibitions that serve as a buffer against what we know is bad behavior.

Rogue Mechanic using my new great crossbow in the 3rd Depths chain quest with a player monk and two cleric hires. We had planned for this quest specifically - making sure we could kill the wraiths. Right before the wraiths, however, I had to do the optional and open the secret doors - as rogues are want to do. Up pops the scorpion and I shoot. My bolt hit's it squarely on the mark and does a lot of damage. That's when I realized it was a champ and I had just grabbed massive aggro! Panic immediately set in! I did my best to back-pedal and shoot, but I saw the writing on the wall. Despite now being prepared for the inevitable, that scorpion killed me even quicker than I had imagined it would.
Sometimes you squish the scorpion, and sometimes the scorpion squishes you.
1st ed: The electricity floor in Snitch... Dance, dance, die. Level 8 holding 9.
2nd ed: arcane skeleton in necromancer's doom. Lvl 2. We will shall never speak of this again. I blame 1st ed trap death anger for trying to zerg. Lesson learned.
3rd ed: still alive and kicking for now but only lvl 3. Slowly, slowly. Creep, hide, recall if bum squeaky moment achieved!
****ed as hell. Level 14, 2100 favor, died to some BS scaling. I knew Black and Blue was a newer quest so I ran it on hard at 14instwad of elite like every other quest I had completed up to that level. Black dragon hits me with a DoT that does 350 damage a tick, after going through 22% dr from mrr, 10% more dr from barbarian, and 32 acid resist after that. A level 10 hard quest has a fricken DoT that does around 520 damage PER TICK?

Asking to remove champs basically just amounts to, "please make hardcore mode less difficult". I don't agree too much with this sentiment. If you want to get 1750 favor and reach level 20 with no difficulty, then just do every quest in the game on casual or normal. No one is stopping you from doing this.

You have an old enough join date to know that we vets that didn't have to deal with champs
Also didn't have any dungeon scaling for solo play.
Every Quest and every slayer area was scaled to a party of 4 regardless of how many party members were there.
The game was far harder when the level cap was 10 then it is today.
The game has 2 levels of difficulty that have no champs present.
IMO Even with chaps present the game is easier then it was in 2006.

Hell Hound of Cruelty + friends , with dots of Acid and Poison covered the acid but had no poison resist item and couldn't heal through the damage.....this life finding a poison item high on the list. Particularly sad as was the crafter for all my other characters. Now I have a specialized crafter who is not allowed to see the light of day.
I died to a shadow when my stat damage got to 0. I can't recall that ever happening on live as I have tomes, etc. Was at full hp. 6 starting strength with a 5 item and 2 from ship buffs. Oh well, time to reroll.
Yesterday was the death of titans.

I haven't died yet, but have been in party with two characters that died.
One died while recalling from Two-Toed Tobias - drowned while on loading screen. That was unfair.
One died in Memory Lapse - ran to an optional area while the rest of the party was shrining.
Arti 7, banking 8 almost 9.
Decided to go back to do the Depths chain on Elite, with a L7 cleric Hireling (Flower).
2nd Quest. There was an Acid Trap. Unlike many similar setups, there was also a jet of acid that was pointing at the box.
I jumped past the first jet and started to disarm... then everything went purple.

the warforge wizard by name of atrymulligan got melted into liquid ooze by an acid trap in deleras part 2 he can now be found roaming the halls of waterworks tormenting kobolds and adventures alike.
kobold remembers waterworks and still hates you

5 deaths from electrical damage at late level 4 and early level 5, one from a spellcaster the rest from traps
1 death from the trap in Deathhouse at level 11
Probably one of the more unusual deaths I have seen was in Korthos during the quest redemption. In it your party is sent to find Larys Heyton, and he is found in a room with a blue magical forcefield covering a body of water. What is supposed to happen is that the Sahuagin bring this barrier down, attack for a while, and then after your party defeats them the barrier goes back up.
Well someone decided to jump in and cross the water while the barrier was down. It must have been bad timing because before he got to the other side the barrier came back up. The poor fellow then started drowning. I guess his swim skill was really bad because he died before he could recall.
It was definitely not a way I ever thought someone could die in hardcore.

First try: Lightning bolt at level two.
Second try: trap at level 8
Third try: missed a jump and fell into lava with no way out at level 9

1st) Level 3 sorc, foolishly joined STK Elite group, did ok, scored some hits, but then … Stepped on the (one) spike that is still active after the traps are disabled in the fire jet room.
2nd) Level 12 (2ART/10Wi) Soul of Cruelty Spectral Mage ghosted through the wall, critically hit with lightning from behind. Left a vacancy on the Leaderboards.
3rd) Level 9 (2ART/7WI) Fen Cat, Fen Cat, Fen Cat … Oh crud, here come the Sauhagin. Scaling in wilderness areas can be a real killer!
