In my experience the only truly scary thing so far is that **** famine reaper in level 3-4 quests. Cold resist, cold absorbtion, full health, doing it at max character lvl - still 1 shot by a *targeted* spell, no chance of dodging that unless you are around a corner at time of casting or something.

I was playing on my level 4 Eldritch Knight Wizard, who I thought was pretty bulky. We got to the end of the quest the 'Kobolds new ring leader' on elite.
This was where it all ended for my character. We were at the final room of the dungeon; the point where the party was set to confront the red named ogre that I assume was the 'kobolds new ringleader'. Everyone stopped right in front of the final door, so I dropped about 3 webs in anticipation for what would be come running through. Crowd control would be my contribution.
Well the party opened the doors and then literally nothing happened. Crickets. No one moved for about 20 seconds. The webs I put up were about to disappear and I was getting impatient. The webs were also blocking my view and whatever was inside must have been around a corner because I could not see anything. So I couldn't figure out why no one was moving.
I didn't know this quest so I decided to run through the doors to prompt everyone to start moving. Well that was a mistake.
As soon as I went through I saw the ogre everyone else must have been looking at. It came jogging towards me. I tried to run away the moment I recognized what it was, but then I saw it wind up the 3 swing strike that ogres do. And then before I could even blink my character was in the land of lost souls.
It gave me an appreciation for how sudden deaths must be for these adventurers. Just a black out and then next, waking up in the land of lost souls.
Other deaths I have witnessed:
Mostly the teams I am have run with have had very bleak survival chances. We have discussed how we often start out with 6 or so people, and the after a few quests we will be down to half. Whenever someone dies we press f to pay our respects. I am not sure if this is serious or a joke.
A lot of the deaths I have seen have been to traps. Usually I will say something like "trap around that door, be careful", and then someone will run into that trap and die.
We weren't prepared for the Reaper cushions.
Hopefully they remembered to claim their reward before dying.
As for me, first toon, Level 2, Rogue/Ranger in Korthos Wilderness area. Slipped off cliff but survived with -1 HP...and nothing to revive with so helplessly watched as stabilization rolls failed one after the other until *ding*. (Oy, "It's just to die from something as petty as 'slipping on tea'...".) I didn't bother to run the "Ol' Sully's Grog" quest, 'cuz, you know, rez cakes won't bring you back from death. However, the incapacitated pop-up also has a link to buy a rez cake with shards, so...if a rez cake can bring one back from incap, I'll not be making that mistake again!

First death? Up until then, my cleric was all about helping the party and other. Healer intim-tank gladly taking hits and spells to keep others alive.
Then, at level 6, I decided to go get a VoM and run Delaras. The goal was to just get through the quests, even if I needed to run on casual. I put up an LFM and asked the guild, but then figured I could solo it. First quest is level 5, so no problem for a Cleric-5/Fighter-1. I could heal, buff, and utterly destroy undead. So, Elite it was. I made it to the first shrine and rested to recover my Turns. Another member joined and made it to the shrine as well. I buffed them up and made sure to heal them as they jumped passed the Cold trap just beyond the shrine. They made it! With my tower shield and plate armor, I used my Anger's Step to cast Expeditious Retreat, and ran through. Somehow - two jets of cold hit me three times.
I wasn't killed by the trap. Greed and vanity did this toon in.

Another one bites the dust. I had high(ish) hopes for this one. Lvl 5 (4/1 Wiz/Rog) incapacitated in WW part 2, Death aura still going but doing nothing when incapacitated apparentlyThen the final Lightning bolt from a Shamen finished me off.

Level 11 holding 13, was running Necro 2 on a pure monk with nothing in the str stat. Taint of Shadow "You have been touched by a shadow, and will die and spawn another shadow, if your Strength is reduced to 0." I had to keep swapping my deathblock item with my underwater action item. One such time I did not swap back in time and got hit by a shadow and immediately a ray of enfeeblement. Ding! Too weak to continue I went to the land of lost souls without taking any damage, just instant death. Had every quest 1-6 done on elite (including chrono) and almost hit the first milestone reward but it all came to a quick end. Moral of the story, don't ever unequip deathblock.

Was killed by traps protecting levers to open the door to the end fight in Sacred Helm.
Was on a lvl 5 teifling warlock, soloing it on elite.

Hardcore Seasons 1, 2, 3 & 4 survivor
1st - Level 4.3 - Deep Gnome 2 Rogue/2 Wizard (Eldritch Knight) - died from multiple Kobold Shaman Lightning strikes in a Reaper Butchers Path.
2nd - Level 2 - Gnome Rogue - died from frost jets in Stopping the Sahuagin. I was standing too close as I was searching for the trap boxes, doh.
3rd - Level 3 (So far) - Deep Gnome 2 Rogue/1 Wizard.
The best part about this is all you have to do is put up an LFM and it will fill ASAP.
So I walk into "Come out and Slay" late..
I'm at the entrance by myself, and all the mobs (that you run by at the beginning) have been activated.
I panic, and moved deeper into the building drawing even more mobs...
Completely surrounded, can't move... I'm fighting desperately... heart pounding (seriously)... I start spamming Blood Tribute...
I hit Blood Tribute 11 times, at -1 CON each time before the rest of the group showed up and rescued me... (I started with 14 CON, so I was very close to death).
Could not believe I survived that... So stupid.
Put some stuff in the shared bank before I logged off and went to bed.
Next day, first quest, walked right into a Poison trap in Redfang and instantly died...
Wish I had put more stuff in the shared bank (I'm really missing a sweet +4 Spot/+4 Search goggles I had found in a random chest)
First death = Stopping the Sahuagin level 2, I was stupid and tried to time the run in the middle of the frost jets. Second life I did it from the side without getting hit.
Second death = Dirk's level 3, Reaper slapped me down
Third death = poor build, I don't remember where but I tried to make a pure offense build (It was late and I was tired) in the hopes of killing everything from a distance before it could kill me. Don't do this.
