I've done the low level quests so often I can jump over/around/time the traps in my sleep now...
Two Toed Tobias favor cleanup: I jumped into the fire jets that melt the ice. My stupidity. This consumed by free life.
Sorrowdusk part 3: The Bridge of Death. In this case, I blame the game, the trap simply hits too hard AND avoiding it is very tricky.

Lv 6 Warlock taken out by Arcane Skeleton in Drydens Crypt. It double tapped me twice with Scorching Ray. Started with 115 hp then 45 hp then dead.
My 2 mains are Mornyngstar and Seldissan on Thelanis but I also have toons on all servers with those names. Any toon that has Mornyngstar Clan as their last name is most likely me.
2 Deaths so far.
1st - My Monk. Poison trap in The Sunken Sewer. Thought I had it timed. Made it past the first one, and by twitch reflex tried to jump over the 2nd jet. I would have been find If I stopped between them and let the healer heal me between failed saves. I was level 5
2nd. My Rogue. Recovering the Lost Tome. Lightning hallway. Was trying to hit the INT rune to clear. Failed every save. Ran back for heals 3 times. Last one double hit me and failed on both rolls. That one made me the most mad.

That section can be hard, but in case you don't know (because it sounds like you were just eating the hits), the lightning only hits in the middle of the hallway. If you time it right you can easily tumble around the corner where the rune is, where you'll be safe and able to turn it off without too much of an issue.

Warlock level 16 in Against the Demon Queen, a wildman mystic hit me with Disintegrate and failed the save. I was sitting at about 2900 favor.

Alts: Caitlan (Ftr) / Gimlinas (Barb) / Chakmar (Sorc) / Shureyoucan (Monk) / Nessyia (Ranger), Mekkanik (WF FvS), etc.
My first indication of how bad things might be was at level 3 on my first character (a Ranger/Rogue) when a moment of inattention in the Borderlands resulted in me going down to -11 hp. One of the other party members had come to help but had no healing, so he basically was watching me bleed out until I got a lucky roll and recovered (the other guy, who was a cleric, never even tried to help).
I had run into 2 gnat swarms while finishing another fight, and that was what brought me low. Yes, I very nearly died from bug bites.
My two real deaths have both been death by reaper. Both at level 6 running a quest that was probably too much for my level and gear.
I have learned, though. It turns out that a shared bank is a very practical investment- but only if you regularly deposit gear within it (gear that may not fit your current character but could be potentially useful to another).
I haven't died yet this season (knock on densewood) but the first season I got my ranger to level 6 and died in the 'Depths' chain to a violet slime - or rather, about a million violet slimes. For some reason, I didn't think the tempest ability that adds electric damage would include unarmed combat, but it does, and before I knew it I was completely surrounded by slimes and was in red alert status. It was actually a very slow death as they didn't do much damage each, but I couldn't run away and non-monk unarmed attack speed is tragically low. Death by a thousand splurks.

Unexpected bunch of epic guys with Meteor Swarm in Heroic Orchard. Luckily I had a free life and was able to eventually make it to L20. Moral of the story...be very careful in Orchard...especially running round corners into bunches of guys that really should not be there!

Died 3 times now - each time it was toward the end of a long session, and I was tired and not paying the attention I should have. It's like snow skiiing - you always get hurt on the last run of the day.
The last time was early in the character life, on a fast run of the last quest in STK, and I thought someone had the last traps (cold jets and spikes) - but the barb had zerged thru without waiting for that, and the trapper had stopped when he saw that the barb was through. I shoulda seen that, but didn't - oops.
