Couple of extra things:
Tomb of the Shadow Knight: Noticed the last couple of times I did this: you need to purify all 4 altars before killing the boss, but you get the DM text after purifying 3 of them telling you now is the time to strike, then nothing after 4 when the mummy does wake up.
Arcane Archer (elf if it matters): Tier 5 ability Final Strike has a different icon in the hover information to the displayed tree, and it uses this icon with octagonal passive borders when you take it to a hotbar, despite being an active ability.
Black and Blue: the quest-giver has a typo of "Kyhber" somewhere in the text.
Nistafa on Khyber
A couple of things I have not seen mentioned here so far.
- Cosmetic weapons does often not show up, very often when a character not in your line of sights swaps weapons. When you see that character one of the weapons will not be visible. Also a big problem on my artificer, where I do not see my heavy repeater myself most of the time.
- The traps in Grim and Barret are also turned around, and does not show as open after disabling.

So they released an item with the sole purpose of allowing players to purchase them for use in customizing the appearance of their weapons, armor, helm, and goggles, and then deliberately removed some very good looking items from being able to be used with them.Mirrors of Glamering should no longer be able to be used on any quest-specific items that are destroyed upon quest exit.
I have a feeling part of the reason might have been to prevent people from glamering the glowing scepter in Rainbow in the Dark, since that would effectively remove the need to have the scepter equipped, and everyone would be able to have one. Also, and probably a lesser reason, people would be able to have a light source in every other quest.
Trap the Soul spell still requires spell components when you have the Eschew feat.

Is there any chance of making the tiny little arrows on the skill screen when leveling up larger? I can't be the only one that is bothered by how small they are, half the time it takes me 2 or 3 clicks to hit the arrow.

I apologize if this one has been addressed already, I did read every post but did tend to barely skim the long-winded ones.
The Demonweb quests can be taken and done in any order, but if you follow the story flow, an error appears.
If you do Trial By Fury first, then turn it in, Xarathas says: "While you were away we received information about a second arcane Orb...Speak with Ymris. He has a proposition for you."
The first player response is: "I'll go talk to Ymris about retrieving a third Orb."
Obviously this should say second Orb, at least for the sake of consistency.

I don't know if this is easy or hard, could you change the shared bank so it displays the most recent items first instead of the oldest? I generally don't remember names, or there are several items I need to pull out and having to click next page a bunch of times gets annoying. (Especially when the bank decides to randomly close itself and I have to start all over again.)
Can the Leaves of the Forest and Livewood Core be made fixable via Stone of Change already? It's been like four months since they were (as far as I'm aware) the only items left out of the Druid's Deep items that were being fixed.
Also if it hasn't been issued already, the Captain of Bling hat turns invisible every time you log out and log back in.
And I'll just casually mention the lost chest in Acute Delirium that we haven't been able to reach for like half a decade.
Just a random, forgettable NPC doing things an NPC does
Things that NPCs do include, but are not limited to:
Having a gold goblet over my head to indicate availability of a quest
Having a catchphrase that never really catches on
Having various rewards that are generally not worth the trouble
Wandering around randomly
Giving out obvious information if it wasn't obvious enough
Repeating the same lines over and over again
Repeating the same lines over and over again
