Has this been worked out completely? What all gets counted into it? I know rogue level, dex bonus... But, what about vertigo or bonus DC to combat feats from dwarf/etc, or from items like consuming darkness? What extras count into the DC?

The tooltip has been screwed up for a long time. (example)

All of it. You can test this in quests by swapping gear around at various difficulties. Pretty much the only thing that won't is the Improved trip feat, as it’s a completely different attack rather than a bonus to your trip attacks.

Here’s the breakdown for my character.

+18 - Sweeping Strikes III base
+20 - Rogue Levels
+25 - 60 DEX
+12 - Epic Boots of the Innocent
+05 - Epic Landslide
+08 - Know the Angles
+01 - Fighter PL
+01 - Tactical Training Room
+06 - Legendary Tactics
+04 - Scion of the Astral Plane

This was good enough to knock EE Amrath foes and LH Tempest Spine on their butts 100% of the time. It seemed to be effective on LE. But, I'll admit that I was too busy making sure I got the hell out of the middle of the fight after using it.

Also, if you’re adept at using your tumble keys, you can keep the capstone running. That's on vorpal, no save trip mixed with a Tier 1 Dance of Death effect.

Btw, thank you for making the math there i wasnt really sure if my dc were high enough hehe. But atm i should have:

+14 - Sweeping Strikes II base
+20 - Rogue Levels

+15 - Enhancement bonus (Epic Light Unending)
+05 - Insigthful bonus (Ring of house Dun Robar. It has +8% dodge too, i likes it)
+01 - Tactical Training Room
+06 - Legendary Tactics
+04 - Scion of the Astral Plane

It might not be as flawless in LE as you claim it is 100 DC, but it sufice good enough whenever im in an "Oh sh*t!" situation or want to momentarily CC a certain caster mob before DPSing him down.

Yes. Epic Bone Crusher. I went Epic Spare Hand at 20 to E Bone Crusher at 28. I would use Consuming Darkness, but I don't have another slot for an INT item.

Like I said, flawless in EE Amrath. Was a little too busy not getting squished in LE! It's still way better than Lay Waste's DC.
