What is going on here? (MoD raid)

Ran MoD about 200 times across characters, and I've never seen this. Did not die in the run, finished out, and.... this. Graphics bug?
Logging out and in resolved it, but I'm curious.

The mark of death is the huge AoE Vol casts in the last phase.

Getting what you see in that screenshot involves dying in the AoE and having some lag during the raise (or the quest completing before you take the raise? Something, something), I think. Haven't seen it in a while.
Anyway, relogging fixes it, yes.
