Here's the results of me using 100 Teas:

55x +10 Royalty Saving Throws
14x +100 Royalty Physical Resistance
10x +300 Royaly Hitpoints Bonus
21x 75% Royalty Elemental Absorption

... Just did a count of how many times I used Audience with the Queen and its about ~160 ...

I think it's a bug for your character then.
If in 100 + 160 = 260 tries the God mode (let's say it has 5% chance, I don't think it's lower than that) has never appeared then you had a really bad luck, which happens 0.00016% of time (since 0.95^260 = 0.0000016) if there is no bug.

Maybe the Queen doesn't like too much diplomacy after all?
Have you tried to be less diplomatic?
Try to drop diplo below 60 maybe?

Like varg said, it can be variance, but heck this is a lot of testing and would be really unlucky.

When have you (E)TRed or LRed the last time? Sometimes that can fix stuff when a character is broken in some regard.

I have over 90 diplo depending on buffs and get it all the time. Don't think that would be a problem.

I mostly get the useless "+10 Saving Throw Royalty Bonus" with 85 diplomacy. But sometimes I'm lucky enough to get +100 PPR

2x Indulgence of the Mad Queen (100% random effect)

Still no "Joy of the Queen" (50% random effect per spell)
