Although unlikely, I'll take a shot at them. Is it ascension chamber goggles section with death pact that went off when you fell from tiles?

#1 is indeed To Curse the Sky, nicely done.

#3 and #4 (I'm grouping these two together as the same quest) are not Ascension Chamber.

#2 is not a public area either. Quest or Raid, close to the Stormhorns.

Quest; both screenshots are from the same quest. These I expected to be tricky.

Couple hints:
Quest location is different from the favor it gives (example: Tear of Dhakaan is located in House P but gives House K favor)
Quest has an epic/legendary and heroic version.

I've seen #2, but don't remember where.
With that hint, I'll guess #3 and $4 are from Grim and Barett based solely on the color of the floor.

As far as #2 goes, most people have seen it; they just don't associate that image with where they saw it.

Sorry, I wasn't aware you were referring to Stealer of Souls (which is a quest); I had thought (for some reason) you were referring to the Titan pre-raid (which is a raid)

Using those clues and looking at Quests by Favor, I think The Archon's Trial also has floors that look like that

Not lost. I just had no guess. Until now.

It looks kind of like the guardian drider's ledge in Beyond the Rift. I usually use Cannith Boots to make the jump and don't really look that way, especially after I've made the jump.
