It's The Ascension Chamber, either for Abbot or for Mark of Death, but with the kind of mob on the ground ( Lady Vol, stoned mob, a beholder on rampage) I'd say Mark of Death.

1) I took this one during the "falling floor" bug (floors fell, visuals didn't show it)

3) This one should be easy to guess, but wasn't easy to take
