Having trouble getting my custom mouse buttons to apply in-game. The buttons are setup fine in the control panel, they work in other programs, but they do nothing in DDO. It's a (crappy old) Logitech MX Revolution.

I ask because unless you are good about hacking that thing apart, it could be that which is your problem. Their saving of the file doesn't always work.
If your file is years old like mine, I'd strongly suggest you blast and re create as sucky as that is. So much has gotten added and probably has not been properly setup and could be causing issues.
I'm also using an older mx revolution. Ddo doesn't recognize most of the buttons. I had to use the setpoint software's advanced settings, set up application specific button settings for ddo. The application that you need to set it up for is called dndclient.exe. It should be in the main dungeons and dragons folder. I then set up most of the keys as keyboard keys that I don't normally use, like the numpad keys / * - +. They can be used in combination with modifier keyes like alt and ctrl for a whole slew of shortcuts that you can use without taking your fingers off wasd.
Note: When running ddo for the first time after an update, I have to close and reopen the ddo client for my hotkeys to work.
I already have the setpoint control panel all set up, including the app-specific section. As I'd said, the extra buttons work as I've set them in all other software I've tried. Just not in DDO.
As for wiping ddo.keymap, just tried that, didn't help.
Thanks for the suggestions folks, but no joy.

Does ddo try to install stuff each time you run it? I had that problem a while back, and as I said, every time ddo updates, the hotkeys don't work that time. I had to reinstall ddo to fix it.
yes to dndclient.exe.
also yes to it tries to install c++ every time. I've tried all the fixes for that listed here, including installing the redists manually, but it still does that. Haven't tried re-installing ddo from scratch... long downloadGuess I'll try that next. Thanks.

And the way you post, I'm certain you know that you'd need to right click the launcher and tell it to run as admin to get through that. Something odd must be happening to you. Registry perhaps?
If you redo ddo install, you'll probably want to clean out your register of DDO before you try to re install it. Sides if it has been more than a year, the re install isn't a bad thing.
I don't have your exact mouse model, but I believe the underlying software is similar.
Is your mode selected properly? Is your DDO profile autodetecting?

No joy after re-install. The shortcut is config'd to run as admin already. c++ still tries to install each time. Setpoint sees dndclient just fine. All other software still sees extra buttons correctly...
It's not gamebreaking, but i'd still like for it to work someday.

I've run as admin with the shortcut not set to admin. I've run as admin with the shortcut set to admin. I've run as admin directly from the folder (no shortcut), and not as admin directly from the folder. I've run with and without anti-virus. I've checked and double checked my Firewall settings. I've de-installed and re-installed my SetPoint control panel. And now I've de-installed and re-installed DDO. No change whatsoever in my results.
edit: oh, and I've also tried with akamai present and with akamai removed.

a) part of the prob is that all the c++ version attempt to DL and install a file with the same name: vcredist.exe . So, it may have been my own mistake as I may have missed one of the many vcredist(1,2,3,n).exe's that I'd DL'd.
b) the other part of the prob is that I had to run DDO once in admin mode after the install, then go back to non-admin for the 2nd and subsequent runs. (bizarre, I know).
c) Yes, the extra buttons now work, provided I stay in non-admin mode. I fear I'll have to redo these steps whenever Turbine fields an update of any kind, but at least I know how to work it now.
On the very very PLUS side, the folks that responded to my support ticket were very patient and very fast with their emailed responses. I'm SO happy to report that I've had a good support experience with Turbine. Kudos to the support team!
