I think in terms of creation at the DDO Turbine studios, the kobold is the oldest monster. It was there in beta and in the beginning of Stormreach. They have to be created first to populate the training tutorials and quests.
But then the first tutorial was a gimpy Iron Golem... but I am going to bet on Kobold.

I would say it has to be the Devourer of Dreams in the Dreaming Dark,
He mentions he fought the Stormreaver, which makes him at least as old as the Truthful one, probably quite a lot older.

The only one Gary Gygax himself draw the picture of? The most iconic Invisible Stalker for sure.
Ofc Goblins and Orcs had been the first, but the Invis Stalker gets it!

I'm gonna go with Horoth or Suulomedes, as they probably weren't born, but just started existing during the creation of the worlds.
Others might be Truthful One or some other undead. Maybe some kind of aberration could be ridiculously old, too (like a giant jellyfish!).

Horroth, Suulomades and Arraetrikos. The most powerful of the devils are immortal...there's no telling how long Harry has been leading Sully on.

The Stormreaver!
Let me tell my reasons as to why:
1) he is badass.
2) He and his story have been with us for an eternity!
3) I don't think the Truhful One is 'old' .. as he gave up on his life to become dracolich. And couting undead as old would be unfair (since they are already dead).
4) The Pit Fiends of Shavarath can't be percepted as aging, as Chronoscope raid taught us ... and the poor Tiefling.
5) Who knows whatever Lolth is, she's ugly anyways.
6) I feel really sorry for Stormreaver because nobody mentioned him

Duergar Blackguards
The Dwarf of course...
Namely the Duergar Dwarves that roam the cavernous expanse of the Black Anvil Mines (BAM) in "A Reclic of a Sovereign Past". Of those Duergar the biggest pain in the playing character's rump, were those Blackguards.
That would be a great quest to have an Epic difficulty added BTW!
