I'm a monk fanatic, so naturally I have built out the centered monk options every way I could think of:
9 monk/8 fighter/3 ranger - This seems to be the best split for fleshies; ranged if you want, or spare feats for exotic weapons and/or defensive feats. 3 druid instead of ranger gives you better self healing, but less dps.
12 fighter/6 monk/2 ranger - Mine is a THF bladeforged with paladin splash, which works very well since I get reconstruct as an SLA, but ranger for TWF and ranged works as well; you lose 10% offhand proc but pick up power surge.
8 fighter/6 monk/6 ranger - arguably the best overall split for DPS, switching from TWF to ranged for manyshot and back, but also the worst leveling split; you have to frontload ranger for xTWF, but then you have to frontload monk to take the form feats at 15 and 18 (or else blow epic feats on them), and if you don't frontload fighter, then you aren't centered with your main weapons. Hence the LR20 from pure ranger suggestion.
Race:
An elf with displacement dragonmark and rapiers would be fantastic, but AP tight; you would have to leave some pretty nice stuff on the table, somewhere. Same with helf and either rogue or cleric dilly. Human/PDK has the low hanging fruit in damage boost and healing amp. Horc and dwarf have the best starting stats. Warforged are OK. Bladeforged are amazing.
I can't speak to the high rogue split, but it seems like it would be more sneak attack dependent.
