Learning toughness from the trainer isn't pleasant, but judging from the icon it's a lot less painful than heroic durability:
Greyhawk Wars for TripleA
The same way a trainer teaches anything- a Musical montage.
in order to learn toughness you are shrunk down to 12 inches tall dresses in a white tuxedo and given a red bow tie then you and 9 other trainees are placed at the end of a long wood floored corridor 4 inches apart in a triangular formation (the cocky one is always in front). then the trainers bowl 10 frames forcing u to get up after every frame. at the end of the game you are reverted to your natural size and can now take 6 ft boulders with ease as you have been toughened. because the league champs never fail to pick up the spare!?
He will beat you with a tea spoon for 3 hours a day, couple of weeks, until you feel no pain.
For rapid training, you take 12 hour sessions.

--ignore his smirk.
Select "Toughness".
admire the once mighty feat,
and frown at it's current evolution.
... then listen to Kruzie tease u about those 18 hitpoints, and how mighty u must feel now.

He teaches you the importance of a balanced diet, which is why the bonus increases as you gain levels.
Makes you walk in the desert for a week barefoot.
No wait, that's the Toughness Feet. Nevermind.

If you don't have an SSD, you should be using DDOPreload (fixes the slow first login issue)
Depends on the class:
Barbarian: Hits you with their axe until you get tougher.
Fighter: Lectures you on wartime strategy until you get tougher. If that doesn't work, see barbarian method.
Paladin: Lords their righteousness over you until you get tougher. If that doesn't work, see barbarian method.
Ranger: Feeds you a secret blend of herbs and woodland plants to toughen you up. If that doesn't work, see barbarian method.
Druid: Feeds you a better blend of herbs and woodland plants than those slacker rangers. If that doesn't work, turn into a bear, see barbarian method.
Cleric: Magically enhances your health with the blessing of their god. If that doesn't work, grab mace, see barbarian method.
Favored Soul: Magically enhances your health with the blessing of their god. If that doesn't work, grab the blessed and holy weapon of their god, see barbarian method.
Artificer: Gives you a crafted potion that enhances your health. If that doesn't work, sics their iron defender on you, see barbarian method.
Wizard: Cast Tenser's Transformation, see barbarian method.
Sorcerer: Cast Tenser's Transformation, see barbarian method. If the trainer doesn't know Tenser's, apply elemental damage liberally in accordance with the barbarian method.
Rogue: Steals your stuff, tells you to toughen up, stabs you repeatedly - variant of the barbarian method.
Monk: The lessons of enlightened masters will toughen you - these lessons are administered via fist to the face. Thus teaches Master Bar Bah Ree Un.

Chips, dips; chains, whips. You know, your basic high school orgy type of thing.
It is a little known fact but they have a mini version of the Crucible dungeon located in HOUSE K. It was a Dwarf who first developed this feat and his name was Weeble Wobble. Weeble designed the original gauntlet now known as Crucible for training the typical young clansdwarf when he or she came of age. So successful was the trails and testing that the Giants bought his patent, training equipment etc and moved Crucible to Gianthold. Now there is only the smaller version for those brave enough to take the feat to task. If you make it through you get the feat and a plaque inducting you in as an official "Weeble".
Oh and for those of who have never taken the feat, just so you know, there is a sign at the exit of the training grounds with the official "Weeble" motto.
"Weeble's Wobble but they don't fall down"
A trainer would follow these procedures:
- Wakes you up in the morning at 5am and asked you to be naked into a cold well and soaked in the cold water for an hour. Water temperature can be any where between 60F to 40F
- After the cold water training the trainer would train you in breathing exercises to breath in fresh air to invigorate the body in a Tai Chi manner for another hour to fight the cold from the cold well.
- You are given a good meal. Kargon brand tasty ham, perhaps?
- Sent you off to hot springs to relax in it for another hour. Water Temperature can be 98F to 120F
- Then the trainer brought you on a level 100 ship and ask you to be the training dummy for an hour and then kneel in a washing plate for another hour.
- Soak in cold water again and then shower off.
- After all those bruises, you are given a good whole body and foot massage and have a cleric of the Silver Flame cast heal on you prior to bed time.

With Great Power Comes Great Responsibility
The trainer gives you tough underpants, and explains that "it is a hero thing".
You know those kobolds in Waterworks? They are fledling adventurers in disguise. Only those who kill an adventurer get awarded with toughness and the title of an adventurer along a complementary memory wipe.
And then the cycle begins anew. May the toughest kobold survive.
