Can not add friends?

Hello all,
Me and my friend are having problems when trying to add as a friend. When playing the game i can see the online players and my friend can on her laptop but when coming to find her in the game she is not showing up and she can not see me at all to. When trying to add her as a friend it says that the player do not exist and it will say that on her laptop to when she try's to add me.

Any ideas why?
As like i can see all the online players but i can not see her or add her in the game.

A couple ideas in order of what I think is most likely the problem.

1) Make sure you are both logged in the same server there are 8 different servers and you can only play with/see people on the same server as you.

2) Make sure you are in the same instance. There is a drop down box on your mini-map, you both need to use the same numbered instance to see each other.

3) When adding a friend you have to use their character name not their login.
