I also see that it is not a quest, but a 'saga' menace of the underdark. Maybe something like the gianthold flagging quest guy where they give you an assortment of loot found there, but it actually notices if you did them all on elite?

Since there's no casual in that list, probably something like this:
Casual: renamed to normal
Normal: renamed to hard
Hard: renamed to elite
Elite: true elite
