Weapon graphics in wrong locations

I have been having this problem on all of my characters since the last update. Weapons and shields appear in the air above my head, or on the floor under my feet. They are not in my hands but otherwise look perfect. Almost like the 3D models and animations are broken in some way. This has gone on for weeks now and is extremely consistent ie always happens no matter whether I reboot or not.

In addition some monsters such as the Lord of Blades don't draw at all, I can't see that character on my screen. I also can't see most of the cliffs in the Menechtarun desert, I can see right through them.

I am running an nVidia 680 in Windows 7 X64 SP1 (high-res textures version of the game) and I tried 3 different video driver versions, DX9, 10, and 11 in game and all of the advanced graphics options both in game and the 3D settings in the video drivers. The game looks very good except for the missing/misplaced items. I never had this problem until recently and am attempting to determine if anyone else is having this issue. I am highly computer literate and at a loss as to why this game and only this game is having these unusual issues.

Especially the LoB issue makes it difficult for me to participate, I just end up heading towards where I see the most characters. He is completely invisible.

I might unistall and reinstall to see if that fixes it, but I assume if a DDO file was corrupt the launcher/patcher would detect it and replace it correct?
