Considering that very few people seem to actually be adding the level information about the monsters, I'm tempted to remove it entirely.
Should I replace it with other info about the monster?
eg is it a rare spawn? Is it a red/orange/purple named monster? Info how to find it/get it to spawn?

The level really doesn't matter to most players, honestly. We get that sort of information out of the quest level. (and as a nit pick, the mob doesn't have a level, it has a CR. )
If it is rare or not would be good. If it is not a rare, we know it is a guarantee.
How to get it to spawn, we have the ddo wiki for. So I'd not try to double your work load.
Maybe an additional generic note of the monster type of where is a good place to farm for slayers?
Such as Kobolds run Kobold Assault.
I was tempted to have the database automatically parse the quest entries into DDOWiki links, so clicking the quest takes you to the DDOWiki page for it.
A sound idea (though to nitpick back, i'd personally say there's far more kobolds in waterworks elite, and you dont have to wait for spawn times).

I can fill out the rest of the Oozes tonight when I get home. I'm at 17 and I know which 3 I'm missing for the 20 required.
Or if you are wanting, you can hunt the rest of them down through the ddo wiki. The only named one missing is glomm (I think that is the name) out of Red Fens, The Last Stand. *goes, double checks wiki... adds*
All of the others are generic slimes/oozes/pudding/ochers.
Just don't add any living spells or cubes.
edit: wait, there was one other single (far as I know) location type ooze. Prolific in the quest though. Blighted oozes. *adds that one while he's thinking of it*

Each subrace has its own add button now, you no longer have to type the subrace in!

Notes:
Sahuagin, warforged and spiders have a tiny problem.
Nab Ren, Galinstan, the Bladeforged Paladin and Wood Spider are separate from the rest of their sub-type. As in Nab Ren is listed by himself and then all the Sahuagin are listed. The same for Galinstan and Wood Spider.
Suggestions:
As for the level, I never look at it to be honest.
Information I'd rather have is rare/spawn info and orange/red/purple.

I did not input them into your current system because all of the ones youare missing can appear in many quests, not just one. Several of the monsters have that issue and I don't know how you want to handle it. multimpule entries for locations or not?
Multipule enteries are good so you can see how many things you could check mark all at once.
I mean The Pit has several Trogs you wont' find any where else, a type of slime you will find other places, one named trog, and one name ooze.

Amalgam [Kortos: Redemption] // don't die
Lord of Blades [Lord of Blades] // don't die
Cutlass [Lord of Blades] // doesn't add
Cannith Guard [Lord of Blades, Schemies of Enemies] // doesn't add
Lord of Blades is God + Lord of Blades don't die + Amalgam don't die => Amalgam is God?
