    I remember there was a quest in House K you had to climb down a ladder to get to and if you weren't careful, you could end up falling off the edge while doing so.

    I can die pretty good by falling off the Vault of Night. Even with featherfall it says "you were killed by yourself" which is the same thing it says when you anger the Mabar, or /death. No damage, but no chance of survival either.

    Speaking of old days, the 2e PHB (on p104) ruled that falling is harmless and that the "abrupt stop at the end tends to cause damage".
