Have never done casual, but I've looted many on normal.
"Farming" may be a strong word. I've lost count of how many I've looted, it's a rare run I don't get at least one.
DDO: If a problem cannot be solved by the application of DPS, you're not applying enough.
I got mine on Hard and again on an Elite favor farm. You can try casual, but remember that all loot rates, including named and otherwise guaranteed items are reduced on Casual dificulty.
The problem is never how to get new, innovative thoughts into your mind, but how to get old ones out. Every mind is a building filled with archaic furniture. Clean out a corner of your mind and creativity will instantly fill it.

Drop rates have gotta be way up on elite though, between a full group saw five drop in a single run once. Do all the optionals though.
Being a recent pack, droprate *is* affected by difficulty. If you can handle elite, do elite - preferably with a full group for more people to pass them.
On my monk life, I was running Normal for xp and the garments, and finally looted one on the 8th time through. The next life, I looted 1-2 every single Normal run. So...yeah, bring some friends along, and it shouldn't take long, or just run it a bunch if needed.
It is usually stupidly easy to get these. There is a chest that pops up in the first hallway between the opening cavern and the stinky pit with the shrine. Iirc, it is the chest that pops up once the medusa arrives and is slain. Every time i go into that quest with 6 people, at least one person gets one, so if you are having problems, just enter with group (or solo), kill kill kill to get that first chest, and if you dont get one, just recall out and reform. Haven't needed to try it, but i assume you can do it in about 5 min per run unless the kobolds really mess with you.
