i think its your earbug trinket, when you use it and it wears off you get exhausted

Yes, if you aren't immune to exhaustion, Xachosian Spell Frenzy exhausts you at the end of its 1-minute duration. In addition, if you don't have disease immunity you'll get some minor stat damage and 3 larvae that restore small amounts of SP over time. Eardwellers are fun.

There's the plate/docent with 'metal fatigue' and all other items from the two madness packs carry minor drawbacks.
