For me, the most infuriating bug, or "feature" in this case, is when I'm in the middle of combat and suddenly nothing is happening... am I lagged? I am bashing buttons and nothing. Then I notice "4444ASDSSDDDAAAAAAAAADDDDDDD112311111111888888887 777777777DDDSSSSSS" in the chat box. SCREAM!!
All because my mouse pointer happens to be hovering over a hot bar. Why in the F*** would I want my text to go in chat just because my pointer is over a res scroll? How about a setting that says Activate Chat Box Only on Mouse Click? Or just be able to disable the enter key as activating the chat box in the Key Mapping menu.
Honorable mention goes to spell failure because I'm not looking 100% directly into the eye of my target. Get rid of "looking at" and replace with line of sight like healing spells for all spells please. Can you imagine if in pen and paper your DM told you that your spell failed because you were not looking at your target? There'd be riots.

Mouse wheel bug, I used to play fps games, so I zoom in/out a lot - quickly (nearly always happens when I use my hireling folder)
Millisecond tooltip bug
Shapechange bug (when You shapechange while running and the character constantly jumping afterwards)
Sticky inventory bug

Basically the everyday bugs
And the cooldown bug annoys me a lot since my main is a WF with a 3 minutes cooldown on weapon attachment.
You aren't in stance but are you centered?Speaking of handwraps.. why are devotion (+1 holy etc) giving me a better to hit than a +3 two-hander (both not in stance).
Centered = flurry of blows = full bab
Uncentered = no flurry of blows = 1/2 bab
throwing water baloons at my feet and the splashes everyone around me but not me.
the degeneration of functions when switching between toons multiple times.. usually requires restarting DDO completely and logging in clean to resolve.
This includes issueslile the mouse wheel, lag, poor interactivity with NC's etc....
Another annoying things..
DDO's insistence on resizing my map window.
All hotbars, examination windows, NPC interaction windows, and other displays are static on my screens.
The map view I try to keep it about 6"x6" on my left screen and everytime I zone it resizes down to smaller random small view dimensions. ..
Please give me a lock mechanism to lock the window size for map view.
Astral shard popups like rez window popups when I die that jump in front of a rez that someone has just cast at me.. even when I check the box to not tell me.. it still comes up.. even in raids when it is grayed out and not usable..
****off with the store/AS popups, I don't want them.. If/when I want to use them I can manually slelect them from the menu bar..
Current so called "fix" that causes my bank to close when I open a bag, the bag to close when I open the bank, the bank to close when someone opens a trade window, the dungon window to close if I move while clicking on an entrance.
The stone of change's annoyingly loud and slow crunching animiation when crunching sibrys shards, come on... crunch as fast as I can click .. and crunch bigger stacks...
also.. Stone of change times out and closes periodically, and wont let me drop in different items when existing stack has finished crunching without closing reopening interface.
and why does walking in knee high water slow me down to a crawl when I have Freedom of Movement.

Crowd-surfing and instant red dungeon alert. Not sure if these are "bugs" or "features", but are definitely annoying.
I thought it just remembers whether you leave it open or closed? That's how it seems to work for me, though I don't usually keep it open so I haven't noticed a change.
I can handle the ladder bug, but the water bug is atrocious. Also its other cousin - the air jet bug (same thing, when using air jets). Thanks to this some quests (e.g. the Pit) are mildly annoying, while others (e.g. Spies) are a major PitA.
If it works like Manyshot, then you should be able to hit it again even while the bar shows a cooldown, and have it activate.

I don't really care about the loss of zooming, but this bug also kills control+mousewheel up/down to switch the active hotbar. Because of memory leaks when switching characters, I don't bother just logging the character out/in, rather this causes me to restart the game. This is especially bad for spellcasters, but even my pure barb characters have at least a second hotbar I switch to a lot. Because of the other problem where you can't have an open bag to transfer something to bank/craft, I have to make a choice on "Do I REALLY want to take that item out of the bag and then restart the game because of it?" Having to make that choice is absurd.
Other bugs that annoy me:
* Hovering over an item on hotbar doesn't bring up description. Have to open inventory and hover over something (maybe only equipped?). Then it'll start showing those hotbar item descriptions again.
* Bringing up the enemy's info screen (via "z") and having them die while it's up takes you out of mouselook mode and switches to the other one.
* If you have an enlarged mini-map, entering the guild ship resizes it. So you have to remember to have the small circular minimap on before entering the ship.
I'm a little bummed about combat in general now.
I'm leveling my main again on his 3rd life, monk, and combat is frustrating sometimes. Mobs jitter around on the screen. Swing swing swing swing and nothing, the combat may catch up, or nothing at all. Buffs at beginning of quests, drinks some pots, sometimes they just don't go off and I have to repeat.
I used to think that running him in the higher levels, with all the epic destiny fluff that's been added to the game, had added some performance issues because of all the extra calculations. +x to whateverthehell for y seconds, stacks up to z times. But I'm seeing this stuttering performance crud in the low levels too.
So then I get irritated, and switch to my higher level FVS or cleric for awhile. Then get frustrated again when 1/4 of their offensive light spells fire, but don't land. No roll, no nothing. This usually happens when they're airborne, as they jump around a lot to fire off their phasers, without slowing movement down. But it's frustrating when avenging light fires but doesn't do anything.
Running my arcane archer? Bear in the high road runs straight for me, I drill him with 4 arrows and nothing lands. No rolls. Nothing. Then I get run over. Not fun!

PPS: Don't get me started on the really small two weapon fighting hit box. With mobs moving around so much more now, the TWF hitbox should be increased to what the monk has. Can't hit anything as it blows by me. Booooooo!

They used to call these "quality of life fixes". they even did some of them. then rebroke them. i haven't heard "quality of life" in a long time. and they wonder why i'm not vip anymore... oh, wait, they don't. they don't care.
All mobs being incorporeal until they are behind you, then materializing. This was introduced somewhere late in 2012 I think. A rollback to about update 14 would improve the game immensely. Someone thought they were smart, they were not.
Tight clean procedural code beats the hell out of object oriented bloat and frameworks and libraries up the wazoo (as Jeets would say), somewhere along the line, some new graduates were hired who cutely refer to the original developers as "the ancients" because they wrote their own code, and the newbs who were only taught oo java in school, just could not comprehend it, then proceeded to f everything up.
