This guy claims the Seker build is the fastest, he mentions what you list, maybe you can see more. I have a TR (formerly 20 fighter) up to level 9 right now, it's intresting. Waiting to use my GS Lit II Longsword's to see how interesting it gets

Probably an Acrobat using a staff (+20% alacrity), under haste (+15%, non stackable with air stance or melee alacrity) and using Haste boost IV (+25% alacrity), for a total of +60% melee attack speed.
For anything else (in melee, ranged speed is trickier), unless I forgot something, you're down to +40% alacrity.
On the TWF side, Tempest I & II increase the chance to land off-hand attack by 10% each, thus increasing your RoA.

That build linked talks about things that were either nerfed, or just never increased attack rates to begin with.
EG:
Flurry of blows - never really increased attack speed. Just ups your BAB to full if your using monk weapons.
Tempest I - seposedly nerfed to not include the +10% speed, havent seen that confirmed tho
Wind sthance - no longer increase attack speed beyond haste.. Ultimate wind stance is 15% - same as haste and non stacking.
Theres very little that ups attack rates permanently these days since the devs decided attack fast makes the servers lag, thus deleted most of it in update5/7.
If your fine with handwraps - they are by far the fastest. As they have an animation that simply has far less delay then any other style.
So a monk with enough fighter lvls to get haste boost IV is it.
Acrobats not nearly as fast, due to the fact quarterstaffs have far higher attack delays, thus get smaller benefits from speed increases like fighter haste boost. So whats 20% on paper, doesnt translate into 20% in the real game.
well depends on what you are counting as fastest speed? number of swing animations or number of hits.
if you are counting number of hits you have to look at the offhand hits for TWF and glancing blows for THF , plud doublestrikes for either, including windstance that gives a speed bonus that doesn't stack with haste but still gives a small bonus to doublestrikes.
Probably go with a hasted , grandmaster wind stance monk as i have one and when playing any other TWF toon i feel like I'm swinging in molasses. The exact same feeling when you are running "in to the deep" in the fens and you are underwater trying to swing.
Have to say though my acrobat with haste and hasteboost going is blindingly fast and she doesn't even have acroII yet. ohh i can't wait for acroIII whenever they see fit to bring that out.

Off hand strikes - 80% with GTWF, 90% with Tempest I, 100% with Tempest II.
Enhancement bonus - 15% with Haste, lots of other lower stuff that doesn't really matter.
Competence bonus - 10% with TA I, 15% with TA II.
Double-strike - 5% with Tempest III, 10% Fighter capstone, 10% with Zeal, 2.5% to 10% Wind stance at 1, 6, 12, 18 Monk
Haste Boost - 15/20/25/30% at 1, 4, 7, 10 Fighter or Rogue.
So let's look at some splits:

Ranger 12 F/R 7 Monk 1 is the fastest I can think of. Unarmed is also the only style where offhand/glancing attacks can really be counted as full attacks. TWF is usually pretty close, THF is not close at all.
Did you include the unarmed attack speed bonus in those calculations?

I realized that I failed to include Opportunist, which changes the following:

80% off, 5.5% ds, 30% HB, unarmed
Speed w/ QD: (10 * 111.3 + 19.4 * 147.5) / 30 = 132.5
Total: 4.10
Rogue 12 (or 13) Monk 6 something 2 (or 1)
75% glancing blows, 8% ds, 30% HB, 15% competence, THF
Speed w/ QD: (10 * 113.3 + 19.4 * 140.1) / 30 = 128.4
Total (counting glancing blows as a full attack): 3.92
Also, the 8/6/6 was not correct, as a level 8 Fighter or Rogue has only 25% Haste Boost.

A new one that occurred to me with Opportunist is:

Which is just a hair shorter than the 12/7/1. The margin to more decimal places is .0236 total attacks per second.
