I am not sure we will ever know for sure if he was being a dolt, or covering up about being a dolt, or being more of a devil than a dolt. However, if the leader had said "I expect you to heal though out the quest" and he had then responded "You ain't da boss o' me".... or "Sure" and then hit bladesworn every time the timer came off...we would have a pretty clear picture.
I think the point is- set your expectations up front, and you won't have to look into the hearts/minds of others as frequently. You will know.

No, when you die in the tent eventually your stone gets ejected into the area just outside the bar/steam tunnels and there's a res shrine there.
I agree with the concept that communication works both ways, especially in raids where the increased party size can lead to blurring of needs (1 divine + 1 bard can often heal). Should he have assumed he could be expected to heal for the party? Yes. Should you have assumed he would do that without saying anything? No. Its the sad nature of pug's.
Btw did you check to see if it was intended or accidental that he hit Bladesworn? I have a 20 WF FvS with bladesworn on my bar (mostly for goofing off) and make sure to move it well away from anything else when I am raiding but perhaps he hit the wrong key or misclicked. It's happened to me before while solo'ing but luckily never in a raid.
He used bladesworn during that boss fight after playing his class to lvl 20. He was trolling you deliberately.
