quite a few ways btw if you're on my server i can toss you a con 6 or a gfl item ^^.
greater false life - +30 hp
con 6 (at lvl 13) - +13 hp over the con 4
toughness from minos - +20 hp (also gives heavy fortification which is immunity to critical hits if you don't have it slotted somewhere.
draconic vitality feat - +10 hp from 150 argonessen favor.
toughnesses! - still rocking that power critical feat? that junky lil great cleave? well. for now you can always switch them out for the instant +15 hp feat of TOUGHNESS! ^^.
thats all i can think of thats "easily" acquired off the top of my head :/

GS - +45 hp can be slotted almost anywhere
eventually as well
exceptional constitution +20-40 hp depending on how many con tiers it evens out.
superior false life - +40 hp (10 over greater false life) can be acquired on some epic items

Madstone was probably the turning quest for DDO that made it clear that casters were going to rule and melee were going to be useless.
If you are dying a lot in Madstone, it really doesn't matter how many hit points you have. The healer will be strapped to keep you alive through the entire quest if you are trying to melee at all without a 60 AC. My advice: run Madstone with 2 casters that know what they are doing. My sorc solo'd the quest (on elite) so easily at level 16 that I have never bothered to run it again.

Ive never had any problems with madstone crater on any of my toons, in fact most of the time it was a cake walk, maybe cause of the groups I was with, idk, but most of them where only as geared or experianced as I was, which is to say not that much.
that's about the right HP for your level.
Madstone is hard. they do lots of damage. Do you have a 100% fortification item? That would be my first question.
As stated above, only through items could you raise that, some easy, some very hard to get.
10 from argo favor is not too hard, but getting groups for that can be difficult.
minos helm is worth it, for 20 hp, however if you have a worthy helm that you need that 20 points can take a back seat.
false life items are great, get up to 30. Very rare drop at your level without endless looting, but gianhold does drop them.
Greensteel items are great if you are a TR, but at 13 it is nigh impossible to find groups to get you into the shroud. Most are too scared to run it before 17 or 18th level.
Just keep adding those toughness enhancements, look for greater false life item, get that +6 con item (another very rare drop at 13th that will take lots of looting).
Auction house is a chance too. but expensive.

100% fortification is just as important as HP, but you have that already.
Another thing that really helps is learning when someone is going to throw a spell at you so that you can move out of the way. Eventually, you'll learn to recognise casting animations and you'll just get in the habit of moving a little to the side. You take 0 damage when the flame strike or disintigrate hits where you aren't standing.
Moving around during combat can also limit the amount of damage that you take. If you run around one enemy while you fight it, you'll only take hits from that one enemy while his buddies run in circles around him trying to chase you.
One last thing that helps limit the amount of damage that you take is the amount of damage that you deal. If you kill things in 3 hits, then they don't have long to swing at you. If it takes you 20 hits to kill something, it has more than six times as long to swing at you.

I agree with Mr Wizard - that's plenty of HP for madstone. Just 100% fort (but you said minos right? So that should be covered)
Do what you can to make yourself harder to hit - I'll assume you've got dusk heart for imp false life? That's 10% of blows not hitting you right there.
I'm surprised your reflex isn't keeping you out of trouble with a lot of the spells too?

What you might be able to add, in red. Not much to improve on at this point, beyond getting Greater False Life and Con +6. If you're on Thelanis I can send you any kind of Con +6 or Greater False Life item you want.
Each additional level of fighter is going to give you 17 HP with that Con.
7 more Fighter: +119
Constitution +6 Item: +20
GS Item: +45
Greater False Life: +10
Exceptional Con (+3): +30
Con +2 Tome: +10
Draconic Vitality: +10
Temp HP: +36
= 583
At that point you're fine. And you can probably madstone, for another +40, and Superior False Life for another +10; then you're in the 600s.
I had the following setup on my melee:
Bracers of Str +6
Gloves of Dex +6
Neck/Ring/Belt of Con +6
Belt of Greater False Life
Cloak of Resistance +5
Robe of Deathblock and Greater Fire Resistance
In that case getting resistance to fire/acid/lightning/cold/sonic 20 (in that order of priority) potions (you can buy them from the Twelve) is great if your casters weren't supporting you with resistance buffs (which, let's face it, is a huge hurt to their mana bar). Though with evasion I can't but wonder how much elemental damage you were taking. Deathward (from the Visor of the Flesh Render Guards clicky) is great for inflict wounds spells and of course insta kills and negative levels. Also get some Cure Serious Wounds potions (or Cure Critical Wounds wands as a UMDing rogue splash) for topping yourself off between fights, something that your cleric party member will appreciate. If you were playing elite, well, I can't blame the incoming damage for being so high. But you shouldn't be having this problem on normal unless your entire party was like level 12-14.
