To be honest, I'm not sure.
The last big thread like this came up not long before the Reaver's Reach came out, and the #1 request was Frost Giants.
Well, we got them. Frost giants with blanket immunities to almost everything, enormous hitpoints, and barely any ability to hurt players at all (well, except for the clerics that cast bladebarriers). They attack once per ~3 seconds, and watching them try to harm you is like fighting in treacle.
I don't have a zerging problem.

I would like to see more variety of critters. I'm tired of spider, scorpion, and the occasional rat.
Pretty much this. I think, particularly in the lower levels, that a lot of enemy types are vastly over-used. In the 1-7 stretch, barring a handful of situations, the only enemies I'm likely to see in any volume are Undead, Kobolds, Hobgoblins, and Vermin (of all shapes and sizes).
Which gets kind of boring. Especially considering that later on, you start running into similar problems (with demons/devils, giants, and even some of the above still haunting you).
Some more diversity would be nice.

I'm sure this will sound dumb, but I'd like to see some quests in which the bosses are actually the hirelings in their spare time. (I say this after having gotten *really* aggravated with my hireling a time or two. I'd love to be able to beat on 'em a little bit in righteous indignation. All the while shouting "What's the matter?! No one healing you???)
I want more Rust monsters! I'm tired of those snotty Warforged thinking they're so superior; for every beholder there should be 10 rust monsters! I want quests full of em, quests like:
Enter the Rust Monster: He eats you; you must fight your way out!
Mired in Rust Monsters: The kobolds are at it again! But this time, they have found an even mightier protector...
Running with Bugs: You must travel to Droaam and perform the sacred ritual as a diplomatic favor for the coin lords.
I dream of Rust Monsters: Rusties invade your dreams, and attempt to steal your treasure; defend your gear and gold!
Rust Monster Invaders!: Just like invaders, but the sewers are full entirely of evil outsider rust monsters, who wear scary top hats.
And the Dead (Rust Monster) shall rise...: A rust monster Lich! With rust monster mummy guardians! What could be more awful!?

The rust monster one sounds hilarious to me.
The hireling one sounds like it would be fun but it should have the side effect of making hireling purchases cost more.

cranium rats (mainly cos i loved them in Torment) - might involve an inter-planar trip to Sigil
Aboleth (could be an interesting enemy in the underwater areas, particularly as a major boss)
I used to be with it, but then they changed what it was, now what's it is weird and scary to me.
Chest shaped mimics that lick their lids in hopes of unwary adventurers, fountain shaped mimics that tempt with coins and gems in their waters (which are actually stomach acid), and armoire shaped mimics that scoop you inside with their doors. Yum!
My vote is for more halflings to fight ... for some reason it amuses me to fight them and I always imagine that it improves my half-orcs golf game.

I would like to see more dragons, maybe have some that shift from being in human form when we start fighting them and shift to dragon after we get them to a certain amount of HP. I would also like to see Umberhulks or other creatures from the Underdark/Khyber.
Hydras where each head and the body is a separate target.
Swarm monsters with PnP Swarm traits (Then Vampires could summon rat or bat swarms and not wolves all the time)
More template use; fiendish is overused, but a good example; applying templates to old monsters so that you're not fighting the same old thing; Ones that come to my mind include Mineral Warrior Baatezu or Minotaurs, Half-Illithid Beholders or other Xoriat demons, and Half-dragon Kobolds.
Someone once posted an idea about a module with corrupted Archons; Love the idea. Maybe the Archons are fallen angels, or they are taking "The Greater Good" too far (Could easily justify seeing Stormreach as a hive of corruption that needs eleminated)
