Rothos here, had the same bug as Mr. Cow, this time was after she left the party, mobs kept spawning and spawning, not time in between.
Also,
Playtime, 60 mins no completion, hate that last part with the mobs runing over the star

Got a named item from end reward and normal loot from last chest, the xp and optionals looks ok

During the last battle appeared three sets of puzzle even this being bugged I managed to complete the quest
This quest seem to have several bugs with the final room and this one is only one example
As one can see - three puzzle set - that overlay each other at about 5° rotated.
As this happened to me it was like a nightmare to solve
An other problem is that even if you solved the puzzle the Hag seem to wait till you finish current mobs before she respawn. Also depending at which time you solve the puzzle and if you directly attack her or ignore her and first attack incoming mobs she start to bug out.

Can you describe, step by step, how did you get the three puzzle sets to overlay like this?
The hag is supposed to wait until you clear the currently active mobs before reentering combat, that's working as intended.
She starts to bug out? How?
Please be more specific. In order to fix any issues in this encounter I need to be able to replicate them, step by step.
The screenshot doesn't tell me how you got into this state.
Sorry, it's a bit hard to reproduce for me either and as it happened to me the first time it was in the middle of combat so it was hard to take a sudden break and recognize what exactly happened as I was just baff in the first second
I dind't used a DD at this point, so you can get to that stage even without.
I however definitely solved already the previous two stages. If I remember correctly the hag was just switching into active mode when she flickered (like if she was undecided to go active or not - bugged out) but at the same time mobs where coming around, so I took down the mobs first. As like she was not happy about not having the attention she suddenly switched back to passive and the three puzzle sets appeared. But I am not certain if the hirelings thought about damaging her to fall back into passive as there was just to much going on to take care of.
I am not sure anymore if there was a firewall and Disco at her place...
I will today try to reproduce it and then report how I got there... but I got the impression it was the combination of the other mobs around and the fact that there is a gap when she switch between active and passive.

It's not fixed. We're having a hard time getting this to happen.

It was really nice seeing a dungeon for people who invested into stealth skills, instead of simply invisirunning (I guess that still would work here, /shrug) or dodging patrols like many of the more recent stealth objectives.Please make additional comments here:
Hag looked cool. I also liked having a bunch of medusa to fight.
Puzzle hurt my head. Doing the puzzle 5 times... very frustrating.
Making the rust monster fly as my archer kited it circling the buttons... Fun!
I also thought it was neat seeing astral diamonds as an option for those of us who hate grind farming.

Hmm Well, even without much of a reward it was neat to try.
Here's what the end fight looked like without fighting anything else in the dungeon iirc - on casual though of course. Is it much different with the optionals or on harder difficulties?
- 4 waves of a named Medusa, with a few standard droaam companions. They seemed to have a lot less HP than Hesstess, but used her stone gaze & the dispel ray thing.
- Hag in between them was less threatening than a harbor kobold shaman. A single respawning Droaam orc while I was doing the puzzles.
- Ended with a few more droaam soldiers, and an ogre riding a giant rust monster.
I hope he's there with the optionals, that guy was awesome lol.
Was not the stupidly difficult fight stealthing the Frame Work quest can be on a class without nukes. 4 or 5 heal-casting colornames... ugh!

Killing all three medusas in the final chamber should result in an extra end-chest spawning.

Doing the optionals would include killing all the Medusas beforehand.
Yeah, the hag is personally almost non-damaging, especially if you've got some normal buffs on.
Interestingly, the end healers in Frame Work only become red named on Hard and Elite.
Ok, so fighting them all at the end produces 2 chests. But doing optionals along the way is kobold + medusa + medusa + brinky + medusa + end = 6 chests? (Some people pass by the Brinky chest without seeing it)
