To get the completionist feat you must have all past lives on a single feat. But you may take the completionist feat on a character that has all past lives at level 3.
Leveling to 20 on your last class with 10 past lives will not make you eligible for the feat
According to the DDO Wiki you must reincarnate once from each class. I've always presumed that you must then reincarnate one additional time. That is because Completionist is a feat in and of itself -- it isn't triggered until your final reincarnation from the last character class.
Because there are currently 11 character classes this means it will be available to you on your 11th reincarnation (your 12th life) IF you took a different class each time.
Edit: See Uska's comment below. Edited to remove the error. Note that you must pay close attention to the class if you are a 10/10 split (although I'd imagine this is uncommon and wouldn't be an issue for most people working on completionist).
