Bugged loot table?

Is it possible for a loot table to be bugged for a specific character?I have ran into the deep quest 10+ times on
normal difficulty and have never seen the elders cap on the end reward.
Its not like it is a rare item or anything and should appear just as often as the other set piece end rewards right?

It's all luck.

The Deep can have 1 to 3 or 4 items & there's quite a few for the game to choose from in that list
