I have exactly the same problem.
    Some further details:
I have three installations of DDO (created by copying the directory) that I sometimes run at the same time with different accounts. I used to be able to log any account into any installation.

At some point, I think it was after an update sometime within the last two months, I started crashing in this way when accepting the TOS. I could still log in one account on each installation, the one that didn't need to accept the TOS again, but any other account would display the TOS and crash on "I Agree" (or on "Cancel" for that matter).
I haven't tried to fix it before now, because, well, I had a workaround - I just had to remember which installation works with which account. But today I tried logging in a fourth account to try something, and it didn't work in any of the installations.

Wife solved my problem: She told me to do a proper install of the game.
Until now I've been running old installations from before I installed Win7. I.e., I haven't actually run an installer on this OS, I'm just running the old binaries directly from the directories (and it has worked fine, apart from this problem).

So I downloaded the full high-res installer and installed the game properly, and now the problem has gone away.
I don't know if that can help you in any way.

    Regards
    /RS

If you're seeing the Terms of Service dialog every time you start, then something isn't
getting updated. Did you install the game as Administrator, then try to run it as a
    regular user?

-sigh- I've got exactly the same problem as the original post-er.

Nothing's changed since yesterday except for the Update 7 patch. And the game launched flawlessly prior to the patch.

    I'm stumped.

The groupbox fix might be renaming the launchusersettings.xml file. Further info here.
It occurs to me that the dice are being rolled by the same computer that's trying to kill me. It knows all the moves - it's just toying with me.

    Talking Found A Fix!

I fixed my version of the problem by re-naming the 'LauncherUserSettings.xml' file as described in the thread linked to in the post just above this one.

NOTE: you have to start the game TWICE for the fix to take effect. The first time I tried starting the game after re-naming the file, the same error occurred. The second time I tried to log in, the game worked just fine.

These fixes only help if you have the .xml file. I did a search and there was no file in there of that name or anything similar
