From what I have read on the forums off-hand attacks should also count for sneak attack damage, however when I am in a group and beating on monsters it seems that only about every other attack does not get the sneak attack damage, leading me to think my off-hand is not getting the sneak attack.

I'm not sure what I am missing and would appreciate any help.

Sneak attack damage applies on any strike (both hands) where you do not have agro (or where a few other conditions allow you to get it anyway, such as blindness).

What you could be seeing is agro shifting back and forth, or they could have partial fortification. (for example 50% fort on a mob makes half the time sneak attacks and crits will fail)

Lorien's correct. Most likely you're seeing shifting aggro.

Thanks for the replies, hadn't thought of the fortification possibility yet. I've been getting so frustrated I was considering making a THF rogue, hell I still might just for fun.
