Just experienced it again on Live ---
--- and my reason for mentioning it here is that on the recent ask the Devs at DDOcast, it was mentioned that the Devs suspected that it was due to some sort of database mishandling related to large numbers of people overloading I/O.
Except that, well ... I just experienced it on the *Keeper* server of the European service, and at this time of morning there were only *16* people online, not including any Anonymous people.
This doesn't alter the fact that the randomness of the bug is inherently non-reproducible unless you get "lucky" with it, but whatever the bug is, I can report that it seems to have nothing to do with large numbers of players online and simultaneously accessing the DB.
