Please explain "Main Hand" & "Off Hand"

In this game, yes. There are no left handed people in DDO.

"Main Hand" is the top weapon slot. This weapon is wielded in your character's right hand.
"Off Hand" is the bottom weapon slot. This weapon is wielded in your character's left hand.

Also, the first weapon added to a weapon set is in the "Main Hand" position, while the second weapon/shield added to that weapon set will be added to the "Off Hand" position.

"Slow Hand" is any non-monk wielding nothing or handwraps.

Your character, however, will always scratch with it's right hand. It is a well known fact that those who are left handed, at least in game, are at a disadvantage and seldom make it out of Korthos without suffering a grisly death.

The right hand bias is so deeply ingrained in the game that it infiltrates the character choices.

For example, try making a character named Inigo Montoya, who was well known for fighting left handed in order to give himself a challenge. You will find that Turbine is so right hand centric that they have blocked that name, and any variation of it.
