You forgot the Litany of the Dead trinket, but that is pretty close to the breakdown I had;

18Base +5Level +4Tome +3Exceptional +3Monk +6Item +1Litany = 40 Wisdom
16Base +3Tome +7Epic +3Exceptional +2Halfling +1Litany +4Stance = 36 Dexterity

If you really wanted a higher armor class, you could pump dexterity to 20, but you would be sacrificing alot(even though you already are by using kamas, etc) out of other major abilities like stength, constitution and intelligence.
