Not a technical issue so you won't get much help here. Sorry.
Now that said I'm going to suggest things (and you won't like the start of it.)
1. reset you keymappings to default.
2. close the game. reaname the key mapping file to something new.
3. restart the game. This should create a brand new keymapping file.
((at this point use something to compare the two files. notepad++ has a built in comparison tool, and the software is free. If they are not the same, the major issue was in your original file and the game was not able to correct for it. I've had this issue before.)
4. remap your keys in the method you want. Do this in parts, close the game and make copies of yoru keyfile at each stage.
5. compare these keyfile steps. It is possible that a singular setting is messing it all up. By finding the one that is at issue, you will be able to fill out a more accurate bug report that could get it fixed by update... 4 is my guess. (update 2 is coming in about a week which means that 3 is in final stage testing. I really doubt this would be priority enough to make it in. So that leaves update 4 at earliest.)
There are known issues with a few rekeying so look those up in known issues to see if what you are trying to do will work.
Known driver issue with xbox 360 M.S. drivers: pulling both triggers results in zero, aka no button pressed. Some programs make adjustments for this, I do not know if Turbine is one such that has done so.
edit: this is based on XP knowledge. I do not know how Vista or 7 act.
