PVP was a fun pastime if you enjoyed melee PvP.
Got turned into MUSH with Glancing blows and Guard-items going off in the pit.
I used to enjoy it, now it's just a memory.

yeah it's more of a side-thought. There are basically a few arenas where it is free-for-all. there are also maps for CTF or death match but these are also very uncommon.

As mentioned, DDO is mainly a PVE instance/raid focused game, and the developers are not going to risk killing PVE ballance for the sake of PVP balance, and well, things are not even remotely close to balanced for PVP.
1 cleric would win a fight against 8 rogues every time.
Wizards can keep anyone dancing and out of the fight for hours if they wanted to, and you cannot get anything to counter it.
So rough summery, PVP is a great way, to kill 10 minutes, but any more then that and you will be banging your head against the wall
