Default Just run the ddo application..

Either from a shortcut or from the game folder(make your own shortcut...

Its the dndlauncher or some application in the folder...

Try exiting and relaunching TDM via the right-click menu of the white "Turbine" icon near the clock. If it still gets stopped though go into Options and check the Network tab and make sure that it is correctly configured there and test with "Use peer to peer" on/off as well.

    On Vista "TurbineInvoker.exe"
On XP "TurbineLauncher.exe" (both will list in the program folder as an "Application" and will be the one with the red DDO shield icon)

That being said though bypassing TDM like that can lead to the "You do not have the current version of the client" error when a patch is released (since on "TDM" installs the game is updated via TDM not via the launcher).

If you descide to bypass TDM using the shortcut to those two game files that's fine but just be aware when an update (even a small one) is released you may have to have TDM do the update first and then once updated you can go back to using that shortcut.
