Default Unrepairable item not from MD?

NEW - The spell Mordenkainen's Disjunction (both the player's version as well as the one cast by monsters) is permanently breaking items worn by players. When the spell is cast on a player wearing items in a worn state, it will flag the item as having been broken (even through the item is not actually broken). This causes the item to become unrepairable. We recommend that players not destroy these unrepairable items.
From what I've read, one need only avoid PvP and the Abbot to avoid this bug. However, I have an unrepairable item on a character which has never participated in PvP and is not even flagged for the Abbot. To the best of my knowledge he's never been hit with MD, but I might have missed it (is that possible?). Is there anywhere else this spell is used? Or has my item become unrepairable through some other means?

    Thanks.

Edit: the item is a greensteel throwing axe (bound) that has one point of temporary damage on it that can't be repaired.

MDj cannot hit you except in PvP or Abbot, so it won't be that.

Items can be prema-broken by suffering so much permanent damage @ the hands of the repair people that they fall to 1 or 0 permanent durability. This is working as intended, unlike the MDj bug (where an item with, say, 40/110 permanent durability left is MDj'ed, falls to -5/65 permanent durability, gets flagged 'broken beyond repair' and then when MDj wears off, returns to 40/110 but cannot be repaired again).
    I don't have a zerging problem.

    I'm zerging. That's YOUR problem.

This is why I bind the stuff I want to keep, (armor/wepaons) so they can't take perma damage. (stone of change, attunement)

Please DO remember, that pnp vs DDO is invalid, unless the PnP your talking about is strictly Eberron Settings. So do everyone a favor and not debate pnp vs DDO, since most D&D pnp Is not Eberron settings.

The item in question is a greensteel throwing axe (tier 2 haste clicky item) that has one point of temporary damage on it that can't be repaired.
