Have you turned mouselook off?
The key used to be 'T' and I dunno the new mappings - check under options.BLACK MANTIS - Sarlona

Also check the "Pointer Options" tab in the Mouse control panel config. If any of the Visibility options in there are enabled try disabling them and clicking Ok. Close out of the mouse properties screen and relaunch to test.
