Here's a few tips on how me and A-O duo/PUG our way up to cap in less than 48 hours.

    Requirements for optimal speed:
    Knowledge of the quests
All vital potions. Inculding haste, a lot of haste.
Up to date gear, +1 at level 1, +2 at level 2, etc.
    Multitasking
    Plat, for house P buffs
Kill as little as possible for +% exp.

    Early levels
You start off by doing all the korthos quests related to the main quest line, and be sure to take the "Anger step boots" and "Anger's gift necklace". Also make sure you aquire the Goggles of Insight as well as Spear bane bracers.

The exp here in korthos is bad, but these items will increase the overall speed at which you can obtain levels.

Note that do not level past level 2, until you are 1 exp from the 4'th level.

Recovering the lost tome: normal, hard, elite.
Should take around 2 minutes each run, and skip all the mobs except the last 3 zombies and you'll gain 7% more exp.

Durk's got a secret: normal, hard, elite.
Should take around 1-2 minutes each run.

Information is the key: normal, hard, elite.
Note that this is only for gaining 75 house P favor after Splinter hard, not for the exp it self.
The exp is bad in here, each run should take around 3-4 minutes.

    Stealthy repossession: normal*5, hard, elite.
This is one of the best exp/minute quests out there, you should be doing the runs in 1-2 minutes tops.

Kobolds new ringleader: normal*3, hard, elite.
This should be done in 1-2 minutes.

Waterworks: normal till you hit level 4
    Pt 1: 2 minutes
    Pt 2: Max 1 minute
    Pt 3: 2-3 minutes
    Pt 4: 1-2 minutes

    Splinterskull: normal***, hard, elite.
Okay, this is where you start getting some real exp. Each part should take around 1-2 minutes.
When you reach the part where you are supposed to obtain a key for the door leading to the second part, you finish it, enter the next part, and exit before finishing the next one. This way, you can now repeat the first part again; and as it barely takes more than 1 minute you should be gaining over 1k exp/min. Do this ~7-8 times or so, and the same thing for the next "run-through" part, and the same for the next. In the end you should be well over 5'th level and thus you can now start drinking sof(+3) and Barkskin(+3) potions; which will be needed for the hard run-through.

On hard, do not repeat anything, just run it all through, should take about 20 minutes. When done with hard, you will now have over 75 house P favor, which means you will be able to buff the buffs that house P provides for 30 minutes. From here on now, I will expect that you will always use them, no matter the quest.

Go ahead and do elite right after, this shouldn't take much longer than 30.

    Deleras
Do a single run-through of the deleras series, including necro. The part should take about, 4,4,4,4 minutes to finish without all too much zerging. As soon as you are done take the Voice of the master trinket, no matter what the other items are. Use this trinket from now on till you hit the cap. It provides +5% exp along with some save and skill boosts.
Do another re-run of deleras, and when you hit necro keep doing it again and again with someone else opening it each time. This will provide well over 2k exp/minute if you know how to run it.
Me and A-O are able to do necro duo in 2 minutes, with any character.

    Up to 10'th level
So now you've done the essensials. I would now advice you to do following quests a decent amount of times on normal, and finish em up with hard and elite.

    Deleras hard, elite
Gwylans (should take max 5-7 minutes)
Cult of the six (with farming on some parts)
The pit (should take max 15 minutes, else don't bother)
Excavations part 2 (farm this one a lot, should take about 5 minutes each run)
Tears (only do normal, and about 3 times if it goes in less than 8 minutes, else don't bother)
Stormcleave (should take max 6-9 minutes, else don't bother)
    VON 1-4 if needed.

By now you should be getting around 10'th level, and as soon as you hit 10'th go for GH.

    10-13
Alright, now it's time for Gianthold till you hit 13'th or 14'th level.

Crucible, 8 times normal, 1 hard, 1 elite (should take about 10-20 minutes each run)
Madstone, 3 times normal (should take max 20 minutes each run)
Trial by fire, 5-8 times normal, 1 hard, 1 elite (should take max 6 minutes)

And after that you shouldn't be too far from 13'th, by now me and A-O usually do all the GH quests on elite one time for the favor, this pushes you well over 13'th and closer to 14'th level.

    Max levels
You should now head for the Vale, flag for the Shroud, and keep farming it 1-4 till you get ransacked, then finish it up with a 1-5 run and get on timer.
This will push you over 15'th and not all to far from 16'th level. Now it's basically up to you. Do what you need and I would personally go for Reavers refuge and try to obtain the perfect armor, or do some vale quests elite for favor.
