I would love to run thorugh a quest where you're high in the sky waging battle with the oh-shyt don't get toss off the cloud, or a well designed mountainous region that has a cool castle where the strongest giants reside in.
Looking back at the D&D games I've played, there is one low-mid level monster I've never seen. I've played a lot of D&D based games and expansions and I'm surprised it's never been implemented (to my knowledge). For the record, I've played Eye of the Beholder, Baldur's Gate, Baldur's Gate II, BGII Throne of Bhaal, NWN, NWN Shadows of Undrentide, NWN Hordes of the Underdark, NWN2, NWN2 Mask of the Betrayer, and countless mods for those games.
I've never seen a displacer beast.
Seems like a fun, challenging monster that doesn't require a full party of elite characters to defeat. It's environment (basically, caves) exists in every D&D game ever made so why no displacer beast???

A sea adventure with Ixixachitl.
I always loved having Purple worms or frost worms burst through the ground or ice to attack an unexpecting party. Don't forget the "swallow whole" special attack

Kraken, or a Mind flayer, vampire
For the kraken, the 1st part of the raid or preraid could be onboard a pirate hunter ship. Basicly pirates assualt stormreach and the raid party has to defend stormreach harbor against the assualt or two or three pirate ship that have broken thou. Then the part give chase against on of the last ship only to be besieged by a kraken.
