Do feats, action point and items stack? For example if I take the spell penetration feat, and use action points on it and then carry a spell penetration item will they stack?

Sometimes. It depends on how they are listed in the enhancement list. Some duplicating enhancements can only be taken under one class heading. Take barbarian haste boost and you can't have ranger haste boost. Others are listed separately because they are not both class driven so will stack. Take elven spot and you can still take rogue spot (but not ranger spot and rogue spot together). Still others seem the same but aren't really. Rogue haste boost is for combat while barbarian haste boost is more general, so you can have both -- they don't stack because they affect different things, but they sound a lot alike.

Can't say for certain because I'm too lazy to research it but if the healing enhancements are class driven and duplicating then the likely do not stack with each other.

Class enhancements don't stack.
Racial and class enhancements do.

That is why elven spot (racial) stacks with rogue spot (class).
That is why Paladin Concentration (class) does not stack with Cleric Concentration (class).
Etc, etc..

They must stack, surely. A benevolent God wouldn't design a system where they didn't stack!
