Just another quick comment here. I think something that compounds this issue (from my admittedly limited experience) is that as soon as something hits you, you leave sneak mode -- even if invisible. This is especially bad with range attacks.
If you are invisible and take damage, you should still be invisible. Opponents might know that they hit you, and where you were at that point. But if you move, they should still have to make an attempt to locate you.

I'm curious to see if this has fixed the issue...The DDO Game Servers are now available following a hotfix intended to address lag issues and some monster misbehavior players have been experiencing. Thanks for your patience while we worked on the issues!My Videos Shadow Mage (ok, it's a build now)

Just tested it. They did indeed fix the issue, as well as another issue with stealth.
I'll leave the other thing they fixed as a pleasant surprise for other players to find themselves.
