Your pingplotter is showing loss very early in your connection. This normally denotes some sort of local connection issue. As far as the next step, I would recommend contacting your ISP with the results of your pingplotter and see if they can help you in any way.
You may also want to run a virus/malware scan just in case, as something definitely odd is going on. Bitdefender offers a free online scan at www.bitdefender.com that is fairly good at catching everything.

Your first hop is a 192 ip address, first thing I would do is eliminate your router. Bypass router, go direct connection to your computer and test it again. Also may consider disabling windows firewall if it is running (or for that matter any 3rd party firewall).
