lets see.. Here are the ones that I can think of off the top of my head where I've noticed myself getting stuck..
starting at the beginning:
Harbor:
- Sully's Basement. (getting out of the hole)
- Millers Debt (only 1 ladder there

- The Low Road (both sides and if you actually use the ladder coming up the pipe before the shaman)
- WW1 (the ladder up to the chest by the first switch)
- WW2 (climbing out of the water coming back from the zombies)
- WW3 (the ladder getting out of the water after the drop)
- Sewer Rescue (the ladder right before the trap and both ladders before the hobgoblin - don't remember getting stuck by the spiders though)
- Garrisons Missing Pack (climbing up either side to the switches and climbing out of the water before the end)
Market:
- STK1 (ladder heading out of the water after hitting the 3 levers, the ladder in the wight priest's room, the first ladder back in (were you to want to go back up, that is) and the ladder between lvl2 and lvl3 - after the locked grate and human necromancer (again if you wanted to go back up))
- RedFang (usually don't want to go back up those ladders, but it happens, also the ladder by the chest after the poison room)
- Swiped Signet (ladder up to the optional party, ladder out of the water by the scorpion)
